Our Practice
Goals
We will provide superior reproductive and other related
medical services for women, men and children. To accomplish these goals, we must
do the following:
- Place our patient's best interests first, exceeding
their expectations whenever possible.
- Utilize ongoing research, high academic & ethical
standards and draw upon our experience with the advanced technologies.
- Provide patient and community education.
As we realize this vision, our greatest
rewards will be working successfully with each other, and earning our

Excellence, Ethics and Experience...
We strive to provide our patients with a combination of
excellence in reproductive medicine, an extraordinary experienced staff of
physicians, Ph.D.'s and support personnel while maintaining the highest level of
medical ethics.
While we do see a wide variety of patients from birth through menopause, the
majority of our time is spent with the subfertile couple. For those particular
patients, we strive to do the following:
- Establish the pregnancy as quickly as possible.
- Obtain the pregnancy in the most cost-effective method
available.
- Minimize the chances for complications (i.e., no
litters...).
- Minimize the use of procedures which offer a minimal
chance
of success.

to our patients and excellence in reproductive medical care. |

Scheduling
Appointments should be schedule as far in
advance as possible. Our regular office hours are 8:00 a.m.-5 p.m., Monday through
Friday, and phone calls are accepted starting at 8:30 a.m. daily. Weekend and
holiday hours are available for follicular monitoring, inseminations, egg retrieval
procedures, embryo transfers and blood testing as needed.
When you make your first appointment, you will be asked simple questions so an
appropriate amount of time can be scheduled for your visit. The receptionist will
contact you a day or two prior to your visit to confirm your appointment. New
patients are encouraged to arrive 20-25 minutes early to complete the necessary
paperwork.
Our office prides itself on running on time which is greatly appreciated by our
busy patients. We are only able to run on time when our patients do the same. If a
patient arrives late, the appointment may need to be rescheduled as a courtesy to
our other patients. |

Cancellations
SRMS reserves he right to bill for last-minute
cancellations or failure to keep an appointment. If you find you must cancel or
change your appointment time, please try to give at least 24 hours of notice, so
that another patient on the waiting list can be seen.

Records
Please have copies of your medical records and your X-rays
forwarded to the office prior to your appointment, as these will be reviewed prior
to your visit. Please understand that if you had an HSG (Hysterosalpingogram)
performed, the radiology facility will have your X-rays and not your
physician. |
After Hours Emergencies
If you have an emergency outside of normal office hours,
please call the regular office number. Your call will eventually be forwarded to
the clinician on call. Please keep your phone line open for a return call. Your
physician will worry about you if the line is busy (i.e., calling 911) and may
choose to have your call interrupted. A physician will always be available to
respond to your needs. |

Routine Phone Calls
Routine medical questions are first handled by the nursing
staff. Please give the nurses a full 24-48 hours to return all non-emergent phone
calls. If physician input is needed, the question will be conveyed and the
physician will contact you after a review of your chart. Your physician may have
a number of calls to return, so please be patient for the return call.
SRMS reserves the right to bill for extended phone calls with your physician
which concern matters that would normally be discussed during scheduled office
visits.

Scheduled Phone Calls
Since many of our patients live outside the area, a
telephone conference call may be scheduled in a similar fashion as a normal office
visit. This time will be billed appropriately, but it should be understood that it
is not reimbursed by insurance companies. |

For
Your Safety
For the sake of your health and the health of others, we
must ask that you do not smoke in the office or on nearby grounds. Even small
quantities of smoke are toxic to the growing embryos being cultured in the
laboratory. |
Prescriptions and Refills
All prescriptions and refills should be requested during
regular office hours. This will enable your physician to review your medical record
and make a well-informed decision regarding your care. Please provide the name and
phone number of your pharmacy, as well as the name, dosage, medication allergies
and the schedule of all medications you are taking.
In the best interest of the patient, narcotics will be prescribed only during
regular office visits and will not be prescribed over the phone during evenings,
weekends or holidays without an evaluation. Any patient needing narcotics during
the off-business hours will generally be requested to go to the emergency room for
evaluation. |

How You Can
Help
Please notify the receptionist of any changes in your
address, telephone numbers, insurance carrier or other pertinent information
changes since your last visit.
We appreciate your constructive comments and we encourage you to offer them in a
productive manner. We are always looking for suggestions on how we can better
serve you.
